Basket of healthcare to which citizens are entitled, specified in the lists included in the ordinances of the Minister of Health issued pursuant to Article 31 d of the Act on Health Care Services or listed in the appendix to the notice of the Minister of Health on the reimbursement of medicines, foodstuffs intended for particular nutritional uses and medical devices, issued pursuant to Article 37 Sec. 1 of the Reimbursement Act.
All public health care facilities providing services within the framework of the public health care system in the member state of treatment and all private health care entities providing services outside the public health care system in the member state of treatment, in particular:
- outpatient clinics, including primary care outpatient clinics and specialist outpatient clinics,
- hospitals, clinics, institutes,
- individual or group professional practices of doctors, dentists, nurses or midwives among others,
- diagnostics laboratories.

POLISH PATIENTS
- a patient – a person having either mandatory or voluntary insurance with the National Health Fund, or
- a patient – a person not having insurance with the National Health Fund, entitled to receive health care benefits;
entitled in specified conditions to health care;
This right is conferred on persons holdingPolish citizenship who:
- reside in Poland and meet the income criterion, in accordance with the regulations on social assistance and whose entitlement to health care benefits has been confirmed by a decision of the head of the commune of domicile (mayor of the town, city mayor),
- are under 18,
- reside in Poland and are in the period of pregnancy, childbirth or the puerperium, and receive health care benefits under the provisions on coordination of social security systems or under the directive on cross-border healthcare.
PRIOR AUTHORISATION OF TREATMENT ABROAD
Authorisation is required to receive guaranteed health care benefits and is issued in the form of an administrative decision of:
1. a competent branch manager of the NHF Regional Branch – an authorisation to receive health care benefits specified in the list of healthcare benefits requiring a prior authorisation of the NHF; obtaining such a decision is the basis to claim reimbursement of the costs of the medical treatment for which the patient applies; (on the basis of the cross-border directive)
2. a competent branch manager of the NHF Regional Branch - an authorisation to receive health care benefits provided in Poland; obtaining such a decision is the basis to receive healthcare benefits financed by the National Health Fund ( on the basis of regulations on the coordination of social security systems)
3. the President of the NHF to receive health care benefits which are not provided in Poland; obtaining such a decision is the basis to receive health care benefits financed by the National Health Fund (on the basis of the Law on Health Care Services).
